The additional page is accessed using a PIN number and is private to you. You can use it add a memory of your loved one, for example a photograph, poem, favourite memory, etc.

Inscriptions in the book of remembrance can be seen on the first anniversary of the date of death. However we can only guarantee this if you send the form back at least 5 months before that date.

A token entry is a temporary entry which will sit alongside the book of remembrance for the correct date, until the formal entry for your loved one is added.
